[pgpool-general: 4470] Configuration pgpool in Ubuntu

Hi All,

I am configuring pgpool over ubuntu, here are my settings:
Ubuntu 14.04.3 LTS
postgreSQL 9.3 on both databases
pgadmin 3.3.1 on cluster
And I am following this link
<http://www.pgpool.net/pgpool-web/contrib_docs/simple_sr_setting2_3.3/> as
a guide.
I've already done every step of the tutorial, but my database still up but
disconnected:

I am able to connect all machines through ssh without password. pgpool.log
does not show any error (attached). My pgpool.conf is basically the same as
the shown in the tutorial I just have changed the IP's.
Any tip why it still disconnected?
